What Is Greater Trochanteric Pain Syndrome?

GTPS covers two tissue problems near the greater trochanter, the bony knob on the side of your hip.

One is gluteal tendinopathy, or wear in the hip tendons. The other is trochanteric bursitis, inflammation of the fluid sac that cushions those tendons.

Women between 40 and 60 make up the largest patient group. Runners, cyclists, and hikers also develop GTPS from repeated hip loading.

When to Consult Dr. Shore About Your Outer Hip Pain

These signs warrant an evaluation:

Your visit pairs a hands-on exam with an imaging review. Dr. Shore walks you through every option afterward.

How Dr. Shore Treats GTPS

Treatment starts with what your tendons can handle. Therapy, injections, or surgery follow in that order as needed.

Most patients get better without an operation. Common choices include:

  • Activity changes like avoiding crossed-leg postures
  • Targeted physical therapy for the gluteus medius and minimus
  • Anti-inflammatory medication during flare-ups
  • Image-guided corticosteroid injections for quick pain relief
  • PRP (platelet-rich plasma) injections drawn from your own blood
  • Shockwave therapy for tendon problems that resist other measures

Many patients notice pain relief within three months of the injection. Improvement can continue up to a year afterward.

Full-thickness tendon tears sometimes resist rehab. Dr. Shore can reattach the torn tendon to the bone through an open or endoscopic procedure, using suture anchors to hold the repair.

Most patients go home the same day with a brace and crutches. Recovery has three stages.

Phase one covers six weeks of gentle motion. Phase two runs through month three with progressive loading. Phase three starts around month five with return-to-activity training.

What Causes Greater Trochanteric Pain Syndrome?

Common causes of outer hip pain include:

Most patients recall weeks or months of outer hip soreness before the pain became unbearable.

Benefits of Early GTPS Evaluation

Catching GTPS in its early weeks often spares you a longer recovery. With prompt evaluation, treatment can:

  • Calms outer hip pain before it disrupts sleep
  • Halts tendon wear before more fibers give way
  • Restores walking comfort and stair tolerance
  • Keeps bursal swelling from turning chronic
  • Lowers the odds of needing surgery later

Chronic cases still respond well to targeted therapy and injection options. Surgical repair stays available when the gluteal tendons tear through.

Frequently Asked Questions

Many patients notice pain relief within three months of the injection. Progress can continue up to a year afterward, though results vary by tendon quality.

No. GTPS affects the tendons and bursa outside the hip joint. Hip replacement addresses arthritis inside the joint, a separate condition.

Yes. Most patients continue moving with a few adjustments. Dr. Shore often swaps running for cycling or swimming until tendon pain quiets down.

GTPS usually appears on one side first. Uneven walking patterns can load the other hip over time, so symptoms may spread later.

Surgery helps patients with full-thickness gluteal tendon tears that haven’t improved after six months of conservative care. Most GTPS patients never need an operation.
